Infertility and IVF Financing Information at the Reproductive Medicine Institute
Most major insurance carriers are accepted (Aetna, Blue Cross/Blue Shield, Cigna, Humana Health Plan, and United Healthcare). In
fact, self pay patients are also welcome. If your health plan is not listed, please call our office. As a courtesy to all our
patients, we will file your insurance claims to the corresponding health insurance carrier. However, you are responsible for all
charges generated by your medical treatment at the Reproductive Medicine Institute.
All patients are expected to present a current insurance card at each appointment. All co-payments, co-insurance, deductibles
and past due balances must be paid in full at the time of service. All patients should be prepared to pay for all charges at the
time of service. We accept cash, VISA or MasterCard as methods of payment.
If your insurance plan does not pay RMI within 30 days of service, you will receive a bill. It is the patient's
responsibility to address any disputes regarding insurance payment or non-payment for your medical treatment directly
with the insurance company.
If your insurance plan has assigned you a primary care physician (PCP), a prior authorization or referral is required at
the time of your appointment. Please be prepared to furnish such information when you check in at our office. If for any
reason you could not provide this information, you will be asked to either reschedule your appointment or to pay the visit
in full. If you opt to pay the visit in full, you will need to sign a financial waiver taking full responsibility for the
payment of your office visit or treatment. Thus, your insurance carrier will not be responsible for payment of such visit.
If a refund is due to you, it will be released only after the insurance carrier has processed all your claims.
For patients in need of financing any infertility treatment, RMI has partnered with both Capital One's Family Fee Plan and Care Credit.
